Why PPOs are scarce in Florida’s individual market

Carriers control costs by negotiating with a smaller set of hospitals and doctors, so narrow HMO and EPO networks are the norm on the marketplace. In a year when Florida’s proposed 2027 individual rates are up 15.3% on average (unweighted), carriers have even more reason to keep networks tight. The 2027 carrier lineup has also changed: Cigna, Molina, and Sunshine State Health Plan are leaving Florida’s individual market. Florida Blue says that for 2027, BlueOptions PPO “offers the greatest flexibility to choose the providers you want, including out-of-network (at an additional cost).” It describes BlueOptions as its largest network, with no referrals required and coverage when you travel out of state (Florida Blue). Florida Blue notes that some plan details on that page still reflect 2026, so confirm the 2027 metal levels, deductibles, and county availability when plans post for Open Enrollment, which runs Nov 1, 2026 through Jan 15, 2027. Off-exchange and group PPO options

  • Group plans: Small-business owners can often offer PPO options through a group plan. Cigna is leaving the individual market, but it still sells employer/group products.
