Health Insurance for Florida Snowbirds and Second-Home Owners: Nationwide Coverage Options

Most ACA plans in Florida are built around local networks and cover only emergencies when you are out of state, so snowbirds and second-home owners need to choose coverage deliberately. Under 65, the best fit is usually a PPO with a national network (such as Florida Blue’s BlueOptions, which uses BlueCard when you travel), bought in the state where you actually live. At 65 and older, Original Medicare plus a Medigap policy is often the most portable setup.

Key takeaways

  • ACA plans are local: They are sold by state and county, and most HMO and EPO plans cover only emergencies out of area.
  • Residence decides where you buy: You generally must live in the state, and in the plan’s county, to enroll there.
  • PPOs travel better: Blue PPO plans give access to participating providers nationwide through BlueCard, but you should confirm what your plan covers for extended stays.
  • Medicare is different: Original Medicare works with providers nationwide. Medicare Advantage plans are usually tied to a network and service area.
  • Moving is a qualifying event: A permanent move can open a Special Enrollment Period.

Why most ACA plans don’t travel well

Marketplace plans are approved state by state and sold by county. CMS says consumers “generally must reside in the state where they’re applying” and must live in the county where the plan is offered (CMS). In Florida, many individual plans use HMO or EPO networks. If you are spending July in Asheville or October in Michigan, these plans typically cover a true emergency but not a routine specialist visit, a planned MRI, or follow-up care. For high earners above 400% FPL who pay full price with no subsidy, paying for a plan that doesn’t work where you spend half the year is costly.

PPO national networks and BlueCard

Blue Cross and Blue Shield companies share a program called BlueCard. Florida Blue tells members that when they travel out of Florida, they are covered under BlueCard and receive in-network benefits, with protection from balance billing, when they use a BlueCard participating provider. Florida Blue’s 2027 BlueOptions PPO page describes coverage as “in-network nationwide & worldwide.” Keep a few points in mind:

  • Benefits still follow your Florida plan’s rules, including deductibles, prior authorization, and exclusions.
  • HMO plans like myBlue are built around a Florida network and PCP, and out-of-area non-emergency care is far more limited. Read the plan’s out-of-area terms before relying on it.
  • BlueCard is designed for travel. It is not a way to buy a Florida plan while living primarily somewhere else.

Off-exchange PPOs are worth comparing too. See our on-exchange vs. off-exchange guide.

Primary residence rules

Buy coverage in the state that is truly your primary home, where you live most of the year, are registered to vote, and hold a driver’s license. You can’t keep a marketplace plan after a permanent move to a new state. CMS says you must end coverage in the old state and start a new application in the new one, and you should report the move within 30 days so you avoid a gap or paying for coverage you can’t use.

Medicare vs. under 65

At 65, the picture improves. Medicare says Original Medicare lets you use “any doctor or hospital that takes Medicare, anywhere in the U.S.,” while Medicare Advantage plans may require you to use network providers in the plan’s service area for non-emergency care (Medicare.gov). Many snowbirds pair Original Medicare with a Medicare Supplement (Medigap) plan and a Part D drug plan. Early retirees bridging to 65 should read what happens to your health insurance when you retire.

Travel medical insurance and telehealth

Travel medical insurance is mainly designed for international trips. It is not ACA coverage, it may exclude pre-existing conditions, and it lacks ACA protections, so use it only as an add-on. Telehealth can cover prescription refills and minor issues while you are away. Ask whether your plan’s virtual-care vendor can treat you in the state where you are physically located, since state licensing rules can limit this.

Moving to Florida permanently: your Special Enrollment Period

If you make Florida your permanent home, you may qualify for a Special Enrollment Period. HealthCare.gov says you may need to show proof of your new address and that you had coverage for at least one day in the 60 days before the move (HealthCare.gov). Acceptable documents include utility bills, mortgage or rental papers, or homeowner’s insurance. Learn more in Can I only change my health insurance during Open Enrollment?

Frequently Asked Questions

Can I buy a Florida marketplace plan if I spend summers in another state?

You apply in the state where you live, and Marketplace plans require you to live in the plan’s county. If Florida is your primary home, apply in Florida and choose a plan with travel coverage that fits how long you are away.

Does a Florida HMO cover routine care in North Carolina or New York?

Usually not. Most HMO and EPO plans cover only emergencies outside their service area. Check your plan’s out-of-area rules in writing.

Is Original Medicare better than Medicare Advantage for snowbirds?

Many snowbirds prefer Original Medicare because it works with any doctor or hospital that takes Medicare anywhere in the U.S. Medicare Advantage plans may limit non-emergency care to the plan’s network and service area.

Does moving to Florida permanently let me enroll outside Open Enrollment?

Often, yes. A permanent move can qualify you for a Special Enrollment Period if you had qualifying coverage for at least one day in the 60 days before the move. You may need to provide proof of your new address.
